| Completed tournament roster (not OSA roster) | ||||
| Team Official Books - approved by district | ||||
| Player Books - approved by district | ||||
| Proof of insurance if required | ||||
| ► | Minimum 3 games in the tournament | |||
| ► | Trophies and MVP awards to be presented after Championship game | |||
| ► | Tournament is classified as inter-district, recreational club league All Star teams | |||
